Vista Cove Care Center at Rialto is a 177-bed skilled nursing facility that provides quality, 24-hour skilled nursing and state-of-the-art rehabilitation services.  Our dedicated and professional service providers are committed to meeting the health, social and emotional needs of each resident.

Affiliation
Vista Cove Care Center at Rialto is Medicare and Medi-Cal certified and contracts with most insurance and other health care providers.  We have contracted, strategic alliances with rehabilitation, pharmacy, dietary and other professional service organizations that supplement the provision of care and services to each resident based upon their individual need.
